Disease management a program for managing chronic diseases. These programs are commonly offered by health insurance providers in order to help covered employees stretch their lifetime coverage amounts and to hopefully return better treatment outcomes. By providing this additional feature of a health plan to employees, employers can actually help reduce overall health costs in the long run through a healthier and more content workforce.

Disease management programs seek to educate patients about their conditions, often provide a hotline phone number to get answers about the condition, treatment, or insurance, from a nurse case manager, and analyzes patient history to find any treatment gaps. The goal is to help people with the targeted conditions to maintain their personal best health and therefore reduce costly procedures and sick days, as well as stretching use of the insurance plan to its fullest before the lifetime maximum is reached as many of these targeted conditions can eventually reach over 1 million in services. Disease management plays a vital role in the employee's health, emotional well being, and the employer's cost managment.
